How a Video Visit Works

1. Find an Open Time

Use our easy appointment link to find a time: doctor video visit. We often have openings on the same day.

2. Appointment Confirmed by Text

You’ll receive a text confirmation which includes a link to start your video visit.

3. Medical Visit

At your appointment time, click on the text link. You can use your phone or a computer/tablet. You don’t have to download any special apps. When the appointment is over, you are always welcome to schedule a follow-up video or in-clinic visit.

4. Prescription Refills

If your provider recommends a prescription, or you need a refill of an existing prescription, we’ll contact a local pharmacy of your choice usually for same day pickup.   • Older Adult Mental Health Services: Mental Health Case Management for Older Adults provides treatment and consultation to individuals 60 years of age and older who suffer from mental illness. For those enrolled in the program, we provide assessment, counseling, placement, Lanterman-Petris-Short (LPS) conservatorship monitoring and linkage to community services.We assess the individual’s problems and needs and collaborate with them, along with their supporters, on the development of a wellness and recovery plan. We then set out with them to accomplish the objectives of this plan. The plan is revised based on needs that arise.Services may include

    1. Case Management: This involves coordinating and managing services for individuals to ensure they receive the necessary care and support.

    2. Family Support or Collateral Services: These services provide support to family members or others closely connected to the individual receiving mental health services.

    3. Individual and Group Counseling: This involves providing counseling or therapy services to individuals, either one-on-one or in group settings, to address mental health concerns.

    4. LPS Conservatorship and Monitoring: LPS stands for Lanterman-Petris-Short Act, which deals with the involuntary commitment of individuals with mental disorders. Conservatorship is a legal arrangement where a guardian is appointed to make decisions for someone unable to do so themselves. Monitoring ensures that the individual’s rights are protected.

    5. Linkage to Resources and Supports: This service helps individuals connect with various resources and support systems, such as housing, financial assistance, or vocational training.

    6. Medication Monitoring: This involves monitoring an individual’s medication regimen to ensure they are taking prescribed medications as directed.
